Registration
All
participants must be registered, and badges must be worn to gain entrance
into all sessions and events.
There will be three ways to register prior to November 11, 2005, as
described below under How to Register.
On-site
Registration: After
November 11, 2005
you must register on-site.
The registration desk will be open on Saturday, November 19, 2005, at 6:00
pm at the Chicago Hilton. You will be able to pick up your badge and APS
Bulletin if you have pre-registered, or register on-site if you have not
previously registered.
Fees
The
fees are indicated on the Registration Form. Note that there is a
substantial discount for registering on or before October 21, 2005.
We strongly urge you pre-register to obtain this discount and avoid long
registration lines.
- All participants must be registered, and badges

- Students should be prepared to provide student
identification at the time of registration to be granted student
registration prices.
- Fees do not include meals or lodging.
Members, Non-Members, Graduate Students,
and Retired Registrants: the
registration fee includes all sessions, coffee breaks, one set of conference
materials, and one ticket to
Sunday's Reception at the Field Museum of Natural History.
Undergraduate Registrants:
the fee includes all sessions, coffee breaks and one copy of conference
program.
Complimentary membership is included in
the fee for these registrants:

APS members
who are not current members of DFD: a one-year DFD membership.

APS Non-Members
and Graduate Student non-members: a one year APS/DFD membership.
Payments

Registration payment may be made by credit card (Visa, MasterCard or
American Express) or check drawn on a U.S bank. All payments must be made
in U.S. dollars.

Checks
for the 2005 Chicago DFD Meeting should be made payable to University
of Washington, which is the organization that will be handling
registration for the Chicago DFD Meeting.

Credit Cards: The merchant name that will appear on your credit card
statement for registration at the 2005 Chicago DFD Meeting will be the
UW Engineering Professional Programs. Please make a note of this so
that you recognize the charge when it appears on your credit card
statement.
- We
cannot assume fees for foreign bank transfers.

Purchase orders cannot be accepted.

Payment MUST ACCOMPANY form in order to be processed.

Forms received without payment will not be processed and therefore the
registrant WILL NOT be registered for the conference.
How to Register

After November 11, 2005:

will be able to pick up your badge and Bulletin if you have
pre-registered, or register on-site if you have not.
Cancellation Policy
The registration fee, minus a $50 processing fee, is
refundable if written notice is received by the University of Washington
prior to November 11, 2005. After this date, it is not refundable.
The meeting hosts are not responsible for any charges or cancellation fees
assessed by airlines.
Refunds will be issued 4-6 weeks after the meeting.
